LIABILITY WAIVER
Student (if a minor, including his/her parents or guardians) further acknowledge the existence of some risk of personal injury in participating in said prescribed course of instruction, and that he/she fully understands that there is a risk of personal injury and is assuming this risk without any liability whatsoever to Bitanga’s Martial Arts Center LLC, its’ owner(s), agents, employees, and other students. Said enrollee, including his parents or guardians if a minor, hereby represents that he or she is physically able to take the prescribed course of martial arts instruction which is rigorous and physically demanding. By signing this form you the Student (if a minor, including his/her parents or guardians) are also allowing Bitanga’s Martial Arts Center LLC to send emails to the email address supplied above and are consenting to the release of pictures and results from school social events, tests, contests and any other events regarding the school to any social media or news media used by Bitanga’s Martial Arts Center LLC.
Sparring Policy/Waiver of Bitanga’s Martial Arts Center LLC
I hereby agree to follow all rules regarding sparring or any contact activity when it comes to wearing the proper equipment which is : Helmet, formed mouthpiece, gloves, kicking boots, and groin protector.
I will not participate in any sparring or contact activity without making sure ALL the stated equipment is fitted and fastened correctly. I will not bring any equipment into any facility occupied by Bitanga’s Martial Arts Center LLC that wasn’t approved by a Black belt instructor or Owner(s) of Bitanga’s Martial Arts Center LLC. To ensure approval of the equipment I will purchase the recommended equipment through Bitanga’s Martial Arts Center LLC exclusively.
I understand that the course of instruction is rigorous and demanding. If there is any medical issue which could be aggravated by sparring or contact activities I will not allow myself to participate until I have been given a written release by a physician, which I will give a copy to the head instructor of Bitanga’s Martial Arts Center LLC to ensure when sparring or contact activities can begin/resume.
I also understand the potential danger involved with contact sports and sparring, therefore I assume all risk without liability whatsoever to Bitanga’s Martial Arts Center LLC, its instructors, agents, or students.
By signing this Liability waiver, Student/parent voluntarily agrees to assume all of the risks and accept sole responsibility for any injury to Student, including, but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that Student may experience or incur in connection with attending classes/instruction at Bitanga’s Martial Arts Center.
